28th Nov, 2021, Hallo Majra, Chandigarh, Transgender community: I was so touched & honored to meet Dhananjay ji & the Saksham team and conduct the session on Breast Cancer, related to early detection & prevention and various factors affecting the cancer scenario in the transgender population. Dhananjay ji’s exclusive interview left a mark in my thoughts & heart & will do everything possible to bring acceptance in the healthcare system & bring justice to the transgender community. Will be sharing the eye opening , motivational & heart piercing interview soon. I am so grateful to Anubha Gupta for connecting me to Dhananjay ji, Shane Bakshi, & the Aashray team, & for your collaboration to execute the mission. My heartfelt gratitude to Dhananjay ji’s team & the Saksham trust for sharing your stories & the invaluable interactions. The Trans community were not aware of the Breast Cancer screening, and also about the symptoms of Breast Cancer and other cancers that can be risk factors with the hormonal treatments. We had discussions on the financial constraints, soceital stigma, lack of awareness and negligence from the healthcare system, the Tans community faces.

Thank you Shane Bakshi & the Aashray team, and it was so great meeting you, Anubha & the wonderful team - Ishita Sahni, Chetan Bawa, Deepika Gupta, Deepika Prashar, Manvi Bhargav, Katneet Kaur and Dr Mann. Keep up your amazing work & so touched with the warmth and affection of the Aashray and the Saksham team & looking forward to our continued work together.

I wanted to also take a moment to thank Kanchan Bhaya, my family, who contributed towards my train tickets from Delhi to Chandigarh.

Dharamkhal Village, Cachar Assam

November 24th, 2021, Dharamkhal Village, Cachar, Assam: Bringing the Villagers, ASHA workers, Anganwadi workers together, to carry the concepts of Early detection & prevention of Cancer & our anti-tobacco campaign forward like a ripple effect, in the Dharamkhal village, one of the 11 Breast Cancer Hub adopted villages in Cachar, Assam, India, BCH is serving since February 2020, for door to door Cancer screening-awareness, Diagnosis, Treatment Management and Counselling. We monitor & manage each suspicious case individually, case by case. We take the suspicious cases to Cachar Cancer Hospital, or Silchar Medical College, for further screening, diagnosis and treatment. The hospitals provide treatment at the subsidized rates under the government aided scheme. BCH covers all the other expenses, starting from transport, purchasing medicines, diagnostic tests, that are needed outside the hospitals premises not covered under the government schemes. BCH team assists the patients from taking them to the hospital, helping with income certificate, AAA card, hospital registration, to patient communication with the doctor, accompanying them in the full diagnosis process, buying necessary medicines, dropping them home with follow-ups & counselling. BCH provides treatment aid to the poor and needy and before giving the Aid, our project Coordinator Sanalembi Devi conducts background verification of the economic status of the family of the patient. We take accountability of each case, helping them from beginning to end.

The villagers are daily wage earners living a hand-to-mouth existence without any saving. On many occasions, there is no person in the family to accompany the patient to the hospital, eventually delays or skipping treatment. We take accountability of each patient, case by case.

The 11 Villages BCH adopted since February 2020: Sundari Part-I, Sundari Part-II, Sundari Part-III, Sundari Part-IV, Boali (Boalihaor), Sentcatherine, Bhorakhai, Bidruhipar, Bhaurikandi-I, Bhaurikandi-II, Dharamkhal

BCH Villages Adoption in India (2020): 15th February – September: We completed door to door cancer screening in Bantarapur & Bhorakhai area. October onwards, we move to SIX more villages.. following up on the suspicious cases in all the Eight Villages under our Supervision.

Villages adopted for Breast and Oral Cancer Screening, teaching signs & symptoms of Cervical Cancer and other types of Cancer, Treatment & Management, Palliative Care & Epidemiological Research Study. We maintain database of each member & we take accountability of each patient.

Impact & Success:

1. Bhorakhai: 484 Families screened (1679 individuals

Bantarapur: 640 families screened (3513 individuals).

2. In both the villages, the population was ~100% NOT AWARE on Cancer signs & symptoms before BCH penetrated. After our door to door screening awareness, everyone is alert of Breast & Oral Self screening examination and have become conscious. Most of the people link their diseases to spiritual or religious myths. Therefore, most of the time for any disease villagers prefer going to local faith healers. In this process they lose their valuable time for proper treatment, ultimately leading to death.

Each & every family of Bhorakhai & Bantarapur have now become aware about symptoms of cancer especially, Breast, Oral and Cervical and BCH team has become the bridge to connect with healthcare system, taking accountability of each patient, their treatment, transport, care, counselling & management.

3. Anti-TOBACCO CAMPAIGN: 1983 (56%) individuals consume tobacco in Bantarapur and 1414 individual (84%) in Bhorakhai consume tobacco. BCH team’s anti-tobacco campaign is helping and the villagers are trying to reduce or eliminate.

4. Suspicious cases: Bhorakhai: 34 oral cancer, 2 Breast Cancer & 18 patients complained of chest pain.

Bantarapur : 6 breast cancer, 31 oral cancer and 5 cervical cancer. We are successful in taking some of the suspicious cases to Cachar Cancer Hospital (CCH) but due to fear of Covid19 infection, many patients are objecting to be taken to the hospital but our team is counseling and trying our best to convince the suspicious cases for further diagnostic screenings at CCH.

5. Covid -19 Response: Bhorakhai: BCH team provided Food relief, masks, disinfectants to the victims of lockdown and we maintained database of 222 families to provide food replenishment.

Bantarapur: BCH helped implement and execute health and safety measures via Covid-19 awareness, mask & sanitizer distribution, enforcing guidelines and helping to prevent covid-19 spread in market places with local vendors and public gatherings.

The six new villages adopted from October - 2020 - Dharamkhal Division of Silcoorie Tea Estate; Bidruhi Par ; Sundari Part-I ; Sundari Part-II ; Bhaurikandi Part-I ; Bhaurikandi Part-II.

TEAM

1. Dr. Lopamudra Das Roy, Founder-BCH (as pro bono & voluntary service)- Principal Investigator

2. Project Coordinator- Sanalembi Devi; 15th February, 2020 onwards

3. Project Field Workers : Bhorakhai (Sapna Pashi) 15th Feb 2020 onwards; Bantarapur (Mimila Chanu) March - August 2020; Sundari-I & II (Hatikhal G.P.)
